Anesteziološki rizik obolelih od opstruktivne sleep apneje
Sažetak
Uvod. Pacijenti oboleli od opstruktivne sleep apneje (OSA) podvrgnuti procedurama pod anestezijom imaju povećan rizik razvoja perioperativnih komplikacija u poređenju sa pacijentima bez ove dijagnoze.
Cilj. Cilj ovog rada je da ukaže na povezanost OSA-e i povećanog rizika razvoja perioperativnih komplikacija. Predstaviti moguće komplikacije i patofiziološke mehanizme koji ih mogu usloviti u perioperativnom okruženju. Pregledati dostupne preoperativne metode skrininga OSA-e i strategije planiranja i lečenja koje treba uzeti u obzir kao deo perioperativnog zbrinjavanja ovih pacijenata.
Metodologija. Pretražene su standardne baze podataka da bi se identifikovale kvalifikovane studije koje su uključivale odrasle hirurške pacijente bez, sa sumnjom ili dijagnostikovanom OSA-om.
Rezultati. Pacijenti za koje je utvrđeno da su izloženi riziku ili sa dijagnostikovanom OSA-om moraju imati individualni pristup zbrinjavanja koji uključuje: izbor tehnike anestezije, upravljanje disajnim putevima i praćenje pacijenta.
Zaključak. Rastuća prevalenca OSA-e i njena heterogenost kao i nedostatak pouzdanih prediktora rizika i dokaza zasnovanih na dobro dokumentovanim studijama o efikasnosti perioperativnih mera predstavljaju izazov za dalja istraživanja u cilju primene adekvatne strategije perioperativnog zbrinjavanja OSA pacijenata i sprečavanje po život opasnih komplikacija.
