Synovial sarcoma of the popliteal fossa

Abstract


Soft tissue sarcomas are heterogeneous group of neoplasms making up to 1% of all malignant tumors in the adult population. The tumor generally appears on the extremities near large joints of middle-aged patients, especially in the popliteal fossa. Our patient presented in February 2014 due to a slowly enlarging, darker-colored swelling in the left popliteal fossa. Upon physical examination, a somewhat nodular, immobile, tender subcutaneous mass was observed. There was no locoregional lymphadenopathy. Patohistological findings showed a high-grade primary malignant mesenchymal tumor, biphasic synovial sarcoma type. The patient underwent surgery with wide surgical excision, followed by radiotherapy treatment. Magnetic resonance imagining follow up after one year revealed tumor recurrence. Neurovascular bundle involvement was detected, but without adjacent bone and muscular invasion and above-the-knee partial amputation of the left leg was performed. The intervention resulted in a remission of the neoplastic process and the patient was scheduled for regular check-ups. Broad surgical resection of the tumor with negative margins was the primary treatment in this case. Mutilating operations are necessary when anatomical structures around the tumor do not allow complete reintervention.

Live and work in city Kraljevo, Serbia. Graduated at the Faculty of Medicine in Kragujevac, Serbia, in 1994, the specialization in General Medicine in 2010 and full time employed a general practitioner in Health centre Kraljevo. In my team there are 1800 patients and I care about their health through preventive and curative. The coordinator in my city for violence against women, also member of Multisector team for domestic violence. Healthcare worker Educator on Domestic violence (Population fund United Nations UNFPA Serbia). Member of the Presidency and Scientific committee of the Serbian Medical Society, General Medicine Section. Writing scientific research papers, so far have 33 published. Investigated the doctor's attitude towards violence against women as well as the barrier in detecting more cases of violence against women. In 2019 became a member of the WONCA special interest group for Family Violence. I currently participate in the project IMOCAFV, supported by WONCA WORLD, WONCA EUROPE, EGPRN, EUROPREV and Vasco da Gama, translating questionnaire and forming a national team as national coordinator. Montegut scholar award winner for Wonca Europe region for 2019. Became a member of EGPRN last year and planning to take part in next EGPRN conference in Goeteborg. Reviewer for abstracts for 25th WONCA Europe Conference in Berlin. The European Primary Care Cardiovascular Society (EPCCS) member also since 2019.
