Primena Taxus stentova u lečenju višesudovne koronarne bolesti srca, poređenje sa bajpas operacijom

Sažetak


Uvod: Dugoročan ishod primene Taxus stentova u lečenju višesudovne koronarne bolesti srca je nepoznat. Ranije studije sa običnim stentovima su pokazale njihovu efikasnost uz značajno viši nivo reintervencija u odnosu na aortokoronarnu bajpas operaciju.

Cilj: Upoređivanje efikasnosti perkutane koronarne intervencije sa ugradnjom  Taxus stentova i bajpas operacije kod pacijenata sa višesudovnom koronarnom bolešču, u periodu prve godine od intrvencije.

Pacijenti i metode: Radom je obuhvaćeno 114 pacijenata kojima su ugrađeni stentovi i 93 pacijenta kojima je urađena bajpas operacija tokom 2004. i 2005. godine. Efikasnost procedura iskazana je brojem ukupnih kardijalnih i cerebrovaskularnih komplikacija (smrtnost, reinfarkt, CVI, reintervencija).

Rezultati: Godinu dana nakon procedura, zbirna učestalost u grupi sa stentovima iznosila je 17.5%,  od čega je samo učestalost reintervencija 11.5%. U bajpas grupi zbirna učestalost komplikacija  je iznosila 14%, od čega su reintervencije činile 4.3%. Nije bilo statistički značajne razlike između učestalosti komplikacija (p=0.486) niti između učestalosti reintervencjija imeđu poređenih grupa (p=0.064).

Zaključak: Primena TAXUS stentova u lečenju višesudovne koronarne bolesti podjednako je efikasna kao i bajpas operacija posmatrano godinu dana nakon intervencije.
