Uticaj metode ranog „kontakta koža na kožu“ na uspešnost dojenja

Sažetak


Uvod: Zadatak javnog zdravlja je podstaći majku da započne i održi dojenje svog deteta. Glavni zagovornici ranog započinjanja dojenja treba da su pedijatri i ginekolozi u porodilištima. Rani kontakt „koža na kožu“ podrazumeva da se po rođenju u prvih desetak minuta, novorođenče postavlja u bliski kontakt sa majkom. Nakon što se rodi i zaplače prenese se i postavi na gole grudi majke, koja je uzdignutog uzglavlja i udobno smeštena u krevetu. Dete je postavljeno u uzdužnom položaju u odnosu na nju, između i iznad njenih grudi. Glavom okrenutom u stranu prati mu se disanje, dok su oboje  pokriveni čistim suvim čaršafom. Osnova mehanizma kontakt „koža na kožu“ je hormonska, kod majke se oslobađa oksitocin, a kod novorođenčeta rastu koncentracije kateholamina. Prva dva sata po rođenju novorođenče je budno, aktivno, osetljivo na dodirne, mirisne i temperaturne simuluse koje dobija od majke zbog čega je ovaj period najbolji za započinjanje dojenja. Prednosti ove metode za majku su: lakše i brže zaustavljanje krvarenja nakon porođaja, ranije izbacivanje placente, veća smirenost i opuštenost, veća samoefikasnost u dojenju. Prednosti za novorođenče su: smanjenje postporođajnog stresa i uznemirenosti, manje plača, bolja kontrola dostizanja optimalne telesne temperature, značaj u uspostavljanju mikrokolonizacije crevne flore, efikasnije dojenje sa ranijim otpustom iz bolnice kući. Zaključak: Primena ovakve metode je jednostavna i ekonomski isplativa, a kratkoročni i dugoročni efekti postižu se kako kod majke tako i kod deteta.
