AKUTNA HIRURŠKA OBOLJENJA KOD PACIJENATA SA COVID 19

Sažetak


Od početka pandemije Covid-19 objavljeno je više radova o organizaciji hirurške aktivnosti u datim uslovima. Cilj ovog rada bio je prikazati objavljenu literature kako bismo pružili suštinske predloge o upravljanju akutnim hirurškim problemima tokom pandemije. Urađen je sistematski pregled najrelevantnijih članaka o COVID-19 i hirurgiji objavljen između 30. decembra 2019. i 30. aprila 2020. godine. Pristup operacionoj sali gotovo je isključivo ograničen na hitne slučajeve i onkološke postupke. Upotrebu laparoskopije kod pozitivnih bolesnika s COVID-19 treba razmotriti oprezno i vrlo kritično, jer glavni rizik leži u prisustvu virusa u pneumoperitoneumu . Tokom pandemije COVID-19 trebalo bi dobro proceniti mogućnost odlaganja operacije dok se pacijent više ne smatra potencijalno zaraznim ili mu prete perioperativne komplikacije. Ako je operacija neodložna, nadležni hirurg mora smanjiti rizik od izloženosti virusu uključivanjem u postupak minimalnog broja zdravstvenog osoblja i skraćivanjem puta pacijenta. U slučaju pandemijskih okolnosti, treba razmotriti otvorenu operaciju.
