PROCENA UTICAJA PROŠIRENE HEMODIJALIZE NA STEPEN UKLANJANJA UREMIJSKIH TOKSINA SREDNJE MOLEKULSKE MASE

Sažetak


UVOD. Proširena MCO hemodijaliza (engl. „Medium Cut-Off“) je metoda lečenja za zamenu funkcije bubrega, koja efikasno uklanja uremijske toksine srednje molekulske mase. CILJ. Rad je imao za cilj da ispita uticaj proširene hemodijalize na stepen uklanjanja β2-mikroglobulina. METOD. Ispitano je 16 bolesnika koji se leče proširenom MCO hemodijalizom. Glavni parametar za procenu efikasnosti uklanjanja uremijskih toksina srednje molekulske mase je koncentracija β2-mikroglobulina u serumu pre i nakon pojedinačne sesije proširene MCO hemodijalize. Za statističku analizu korišćeni su: Kolmogorov-Smirnov test, Student-ov T test za vezane uzorke i Wilcoxon-ov test. REZULTATI: Proširena MCO hemodijaliza efikasno uklanja uremijske toksine srednje molekulske mase. Indeks redukcije β2-mikroglobulina u toku pojedinačne sesije proširene MCO hemodijalize iznosi 70,60 ± 5,88%. Prosečan gubitak albumina u toku pojedinačne sesije proširene MCO hemodijalize iznosi 1,88 ± 1,02 g/4h, a indeks redukcije albumina 4,94 ± 2,49%. ZAKLJUČAK. Proširena MCO hemodijaliza efikasno uklanja β2-mikroglobulin. Indeks redukcije β2-mikroglobulina iznosi ~ 71%, a gubitak albumina je manji od 4,0 g/4h. Ovaj modalitet dijalize sprečava razvoj amiloidoze, ateroskleroze i aterosklerotskih kardiovaskularnih bolesti u populaciji bolesnika koji se leče redovnom hemodijalizom.
