FAKTORI RIZIKA ZA NASTANAK BOLNIČKIH INFEKCIJA KOD NOVOROĐENČADI NA MEHANIČKOJ VENTILACIJI U JEDINICI INTENZIVNOG LEČENJA

Sažetak


Uvod i cilj. Neonatalna nozokomijalna infekcija (NNI) definisana je kao pojava infekcije 48 časova nakon porođaja koja je uzrokovana patogenom iz bolničke sredine. Novorođenčad primljena u jedinice za intenzivnu negu novorođenčadi (NJIN) su pod posebno velikim rizikom za razvoj NNI. Cilj naše studije bio je otkrivanje faktora rizika za NNI kod mehanički ventilirane (MV) novorođenčadi u NJIN.

Materijal i metode. Studija je dizajnirana kao studija slučaj/kontrola sadržana u prospektivnoj kohortnoj studiji. Ispitanike studije čine novorođenčad na mehaničkoj ventilaciji, smeštena u NJIN. Pomoću logističke regresije identifikovani su faktori rizika za NNI i njihov uticaj kvantifikovan.

Rezultati. Naša studija pokazuje da novorođenčad u NJIN na mehaničkoj ventilaciji, koja duže nose periferni venski kateter i provode više vremena u bolnici, češće imaju NNI (OR = 1.091, CI = 1.035-1.151). Novorođenčad koja imaju infekciju prilikom prijema ređe dobijaju bolničke infekcije.

Zaključak. Razvoj NNI u novorođenčadi na MV može se sprečiti edukacijom bolničkog osoblja i smanjenjem dužine hospitalizacije. Učestalost NNI se smanjuje ako su novorođenčad kraće izložena bolničkoj sredini i invazivnim sredstvima.
