Анализа фактора ризика за орални мукозитис изазван зрачењем за назофарингеални карцином и прогностичка вредност ЕГФ и сТРЕМ-1
Sažetak
Циљ: Били смо посвећени анализи фактора ризика оралног мукозитиса изазваног зрачењем (РИОМ) за назофарингеални карцином (НПЦ) и корелације између епидермалног фактора раста (ЕГФ), растворљивог рецептора окидача експресије мијелоидних ћелија-1 (сТРЕМ-1) и болести тежине и њихове прогностичке вредности.
Методе: Одабрано је укупно 124 пацијената са НПЦ који су примали радиотерапију од марта 2013. до новембра 2016. и подељени у студијску групу (н = 68) и контролну групу (н = 56) у погледу присуства РИОМ-а. Фактори ризика РИОМ-а су процењени мултиваријантном логистичком регресијом, а такође је процењена корелација између ЕГФ-а и сТРЕМ-1 и тежине РИОМ-а и њихове предиктивне вредности.
Резултати: Старост (>55 година), пушење, лоша орална хигијена и орални пХ < 7 били су фактори ризика за РИОМ, а употреба заштитних средстава за оралну слузокожу је заштитни фактор (П < 0,05). Ниво сТРЕМ-1 се повећавао, а ЕГФ смањивао са тежином болести (П < 0,05). Ниво ЕГФ пацијената је био у негативној корелацији са степеном РИОМ; комбинована детекција нивоа ЕГФ и сТРЕМ-1 имала је већу АУЦ у предвиђању прогнозе пацијената него само детекција сТРЕМ-1 (П < 0,05).
Закључак: Укратко, старост (> 55 година), пушење, лоша орална хигијена и орални пХ < 7 су фактори ризика за РИОМ за НПЦ. Употреба заштитних средстава за оралну слузокожу је заштитни фактор. Нивои ЕГФ и сТРЕМ-1 су повезани са озбиљношћу РИОМ-а и указују на предиктивну вредност за исходе пацијената.
