PRIMENJIVOST FORMULA ZA IZRAČUAVANJE LDL-HOLESTEROLA U HIPERTRIGLICERIDEMIJI: UVIDI IZ VOJVODINE
LDL-C FORMULE U HIPERTRIGLICERIDEMIJI
Sažetak
Uvod: LDL-holesterol predstavlja ključni parametar u proceni rizika nastanka aterosklerotske kardiovaskularne bolesti. Određivanje LDL-holesterola direktnom metodom nije uvek moguće iz praktičnih ili finansijskih razloga, te izračunavanje putem formula ima značaj u njegovoj procenu. Cilj ovog istraživanja je bio da ispita primenljivost četiri različite formule za izračunavanje LDL-holesterola u odnosu na direktnu metodu kod pacijenata sa vrednostima serumskih triglicerida u između 4,5 do 9,0 mmol/L u populaciji Vojvodine.
Metode: Ovo istraživanje je sprovedeno kao retrospektivna studija koja je uključila 272 ispitanika kod kojih su određivani parametri lipidskog statusa u periodu od juna 2022 do juna 2023. godine. Ispitanici su imali vrednosti triglicerida u serumu od 4,5 do 9,0 mmol/L. LDL-holesterol je određivan direktnom metodom na Alinity c analizatoru (Abbott), a vrednosti LDL-holesterola su i izračunavane formulama koje su predložili Friedewald, Sampson, Anandaraja i Martin.
Rezultati: U ispitivanoj populaciji prosečna starost je bila 52 godine a medijana koncentracije triglicerida 5,48 mmol/L. Statistički značajna pozitivna korelacija postoji između direktno određivanog LDL-holesterola (d-LDL-C) i svih izračunatih vrednosti (p<0,001). Najmanja prosečna razlika je uočena između d-LDL-C i LDL-holesterola računatog po formuli Sampson i sar. (MD=-0,032). Poređenjem d-LDL-C i vrednosti LDL-holesterola dobijenih izračunavanjem, jedino formula Sampsona i sar. nije pokazala statističku značajnost razlika (p=0,240).
Zaključak: U ispitivanoj populaciji pacijenata sa hipertrigliceridemijom, formula Sampsona i saradnika je pokazala najbolje performanse u odnosu na ostale ispitivane te bi njena primena mogla biti razmotrena kod stanovništva Vojvodine.
