EuReCa_Srbija 2014-2021. Epidemiološka analiza vanbolničkog srčanog zastoja-uticaj reaktivnog vremena na uspešnost resuscitacije

Sažetak


Cilj ove studije je analiza osnovnih epidemioloških podataka povezanih sa VBSZ i analiza uticaja vremenskih intervala tokom zbrinjavanja pacijenata sa VBSZ na njihovo preživljavanje.

Materijal i metode: Analizirani su epidemiološki podaci, podaci vezani za vremenske intervale tokom procesa reakcije i zbrinjavanja pacijenata sa VBSZ, kao i podaci o ishodima kod ovih pacijenata, na teritoriji sedam opština u Srbiji. Podaci su prikupljeni u okviru programa EuReCa_Srbija tokom perioda 1. oktobar 2014. - 31. devembar 2021. godine.

Rezultati: Ukupna godišnja incidenca zabeleženih VBSZ na ispitivanoj teritoriji iznosila je 137,43/100.000 stanovnika, dok je godišnja incidenca započinjanja primene mera KPR iznosila 50,16/100.000. U grupi pacijenata sa započetim merama KPR, svedok je iste inicirao u 16,98% slučajeva (godišnja incidenca 8,52/100.000). Povratak spontane cirkulacije na mestu zadesa zabeležen je u 18,71% slučajeva sa godišnjom incidencom od 9,38/100.000, dok je u istoj grupi sa spontanom cirkulacijom u bolnicu primljeno 15,49% pacijenata (godišnja incidenca 7,77/100.000). Medijana reaktvnog vremena hitne medicinske pomoći iznosila je 9,0 (IQR 5,0-17,0) minuta, vremena od postizanja ROSC do pristizanja u bolnicu 17,0 (IQR 12,0-27,0) minuta, a vremena od polaska sa mesta srčanog zastoja do pristizanja u bolnicu 9,5 (IQR 5,0-17,0) minuta. Kumulativna proporcija ROSC-a iznosila je 50% nakon 5,6 pređenih kilometara, odnosno 5 minuta od momenta hitnog poziva. Pojava ROSC-a na mestu zadesa nije bila zabeležena nakon 20 minuta proteklih od hitnog poziva, odnosno nakon 11,2 pređenih kilometara od mesta prijema poziva ka mestu zadesa.

Zaključak: Incidenca VBSZ u Srbiji uporediva je sa istom vrednošću u drugim zemljama sa i dalje prisutnim prostorom za unapređenje po pitanju većeg učešća laika u započinjanju mera KPR. Vremenski intervali tokom procesa reakcije HMP i zbrinjavanja pacijenata sa VBSZ duži su u odnosu sa izveštajima iz drugih zemalja te takođe ukazuju na potrebu za modifikacijom onih faktora koji imaju direktan ili indirektan uticaj na iste intervale.
