Ispitivanje uticaja polimorfizma -318C/T (rs5742909) CTLA4 gena na funkciju bubrega nakon transplantacije

SAŽETAK  

Uvod: CTLA-4 protein (engl. Cytotoxic T Lymphocyte Antigen-4) je molekul koji ima značajnu   ulogu u održavanju imunološke homeostaze. U dosadašnjim istraživanjima studije su nedvosmisleno pokazale negativnu ulogu CTLA-4 u aktivaciji T-ćelija. U strukturi gena koji kodira CTLA-4 pronađeni su polimorfizmi u okviru jednog nukleotida (engl. Single Nucleotide Polymorphisms – SNPs) koji bi potencijalno mogli da mijenjaju aktivnost gena a posljedično i proteinsku strukturu. Data genetička varijabilnost se dovodi u vezu sa akutnim odbacivanjem i odloženom funkcijom kalema kao bitnim pokazateljima uspješnosti transplantacije bubrega.

Cilj rada: Cilj studije je ispitivanje povezanosti -318C/T polimorfizma (rs5742909) u genu CTLA4 i akutnog odbacivanja, odnosno, odložene funkcije kalema kod pacijenata sa transplantiranim bubregom.

Materijal i metode: Ova retrospektivna kohortna studija obuhvatila je 151 ispitanika. Lančanom reakcijom polimerizacije u realnom vremenu (RT-PCR) baziranoj na TaqMan metodi utvrđen je genotip ispitanika koji je dalje analiziran u odnosu na podatke o akutnom odbacivanju i odloženoj funkciji kalema Pirsonovim Hi-kvadrat testom i Fišerovim testom tačne vjerovatnoće.

Rezultati: Kod 151 ispitanika najčešći je CC genotip (80,8%), zatim CT (17,9%) i TT (1,3%). C alel ima učestalost 89,7% a T alel 10,3%. Nije pronađena statistički značajna razlika u distribuciji CTLA4 genotipova i alela kao ni njihova povezanost sa akutnim odbacivanjem i odloženom funkcijom kalema. Ispitivanje povezanosti nosilaca C, odnosno, T alela i navedenih posttransplantacionih komplikacija takođe nije pokazalo statistički značajnu razliku.

Zaključak: U ovom istraživanju nije pronađena statistički značajna povezanost -318C/T (rs5742909) CTLA4 polimorfizma sa akutnim odbacivanjem i odloženom funkcijom kalema nakon transplantacije.
