CILJANA TERAPIJA TEČNOSTIMA U VELIKOJ TORAKO-ABDOMINALNOJ HIRURGIJI
Sažetak
Nadoknada cirkulatornog volumena u torako-abdominalnoj hirurgiji već dugi niz godina predstavalja veliki izazov. Primena minimalno-invazivnog hemodinamskog monitoringa je postao standard tokom ubrzanog oporavka bolesnika nakon hirurgije (eng. Enhanced recovery after surgery – ERAS). Takođe, ciljana nadoknada cirkulatornog volumena je ušla u zvanične preporuke za vođenje anestezije u velikim hirurškim intervencijama. Ipak, njena primena je i dalje kontoverzna usled specifičnih uslova kod pojedinih hirurških intervencija (kreiranje pneumoperitoneuma ili kapnotoraksa kod endoskopskih procedura, otvorenog grudnog koša, primene protektivne mehaničke ventilacije u jednoplućnoj ventilaciji). Savremeni napredni hemodinamski monitoring je postao manje invazivan i pruža niz parametara koji se lako tumače. Poslednjih godina sve više radova govori u prilog primene ciljane nadokande tečnosti u intraoperativnoj nadokandi cirkulatornog volumena i njenog pozitivnog uticaja na smanjenje postoperativnog morbiditeta i smanjenja troškova lečenja. Koristeći dostupnu literaturu, preko medicinskih internet pretraživača u ovom preglednom radu je prikazano na koji način se prevazilaze ograničenja primene naprednog, minimalno invazivnog hemodinamskog monitoringa u torako-abdominalnoj hirurgiji. Na taj način će primena ovog monitoringa biti maksimalno iskorišćena za smanjenje postoperativnih komplikacija i mortaliteta. Individualni pristup bolesniku, hirurškoj intervenciji, intraoperativnim gubicima je ključan kako za odabir monitoringa, tako i za postavljanje ciljeva bilansa tečnosti tokom operativnog zahvata. Ovakvim pristupom se direktno utiče na poboljšanje ishoda lečenja, konkretno nakon hirurške intervencije.
